We are looking for a Spanish/English bilingual candidate for a Family Navigator role in Tulsa, OK. Top candidates will have experience in a public health setting or social work and have a baseline understanding of medical terminology in Spanish. The Family Navigator manages a monthly caseload and is primarily responsible for contacting pregnant women recently enrolled in SoonerCare to promote awareness of and enrollment in prenatal and postpartum home visitation services and early childhood programs. This role involves building trusting relationships with expectant mothers, parents, and caregivers while ensuring families are connected to relevant and appropriate community resources. As programs continue to grow, Family Navigators may also support families and caregivers through later stages of the birth through age eight continuum.

Family Navigator Duties & Responsibilities

  • Create a trusting, comfortable, and inclusive environment that fosters partnership and ongoing engagement with expectant mothers, parents, and caregivers from diverse backgrounds.
  • Triage community referrals and Medicaid/Medicare recipients by connecting clients to appropriate home visitation programs, social workers, case managers, and other community agencies based on client needs and eligibility criteria.
  • Promote and facilitate connections to home visitation and early childhood programs serving maternal and child populations.
  • Educate expectant mothers, parents, and caregivers on prenatal and postpartum support, early childhood brain development, and strategies that promote healthy child development.
  • Refer low-income families and children to medical, educational, and social service providers throughout the community.
  • Support community partner relationships by coordinating referrals and identifying opportunities for collaboration.
  • Attend peer group meetings for networking, collaboration, and care coordination.
  • Maintain current knowledge of community resources and relevant training related to early childhood development, cultural awareness, protective factors, and health-related social needs.
  • Serve as a liaison between families and community agencies and networks.
  • Assist with referral linkage and coordination of home visitation services.
  • Collaborate in the development and implementation of initiatives aimed at improving program outcomes.
  • Maintain complete and accurate documentation of all activities and reports through computerized data entry in accordance with program standards and guidelines.
  • Attend required training related to the duties and responsibilities of the position.
  • Perform additional duties as assigned.

Family Navigator Qualifications

Education

  • High school diploma required
  • Associate degree preferred

Experience

  • Experience working with low-income families, home visitation programs, culturally diverse populations, early childhood programs, or health and social services preferred

Skills & Knowledge

  • Knowledge of community resources and early childhood best practices
  • Strong interpersonal and communication skills
  • Ability to build productive working relationships with community partners
  • Ability to prepare reports and communicate effectively both verbally and in writing
  • Proficiency with computers, smartphones, and office software
  • Bilingual candidates (English/Spanish/Burmese) preferred

Licenses & Certifications

  • Valid Oklahoma driver’s license required

Working Relationships

  • Regular interaction with staff, community agencies, and service providers to coordinate referrals and connect families to services.

Physical Requirements

  • Frequent phone, email, text, and face-to-face communication with community members
  • Regular use of computers, phones, and other office equipment

Work Environment

  • Office environment with moderate noise levels, visual distractions, and limited privacy
  • Ability and willingness to travel locally for home visits, community events, meetings, and training

Hours

  • Monday-Friday 8am-5pm
