JOB DETAIL

We are seeking a Spanish/English bilingual Medical Outreach Specialist for a fantastic opportunity in Tulsa, OK. The Medical Outreach Specialist provides health-based outreach services to maternal and child health populations to reduce infant mortality risk factors and address barriers to prenatal, pediatric, and inter-conception care. This includes assisting clients with insurance/Medicaid enrollment, connecting families to healthcare services, and conducting basic needs screenings. Experience in case management and/or medical front office preferred. This is a 6-8 week temporary assignment with the possibility to go longer or permanent in the future.

 

Essential Job Functions

  • Facilitates enrollment and retention of clients in preventive health services by providing timely follow-up to in-person and phone requests and conducting home visits in at-risk areas of the community as needed.
  • Locates and enrolls individuals and families in need of SoonerCare and assists with identifying a medical home and access to healthcare services through community outreach, clinic contacts, canvassing campaigns, and agency partnerships.
  • Screens potential clients for basic needs to determine appropriate Maternal and Child Health services or community resources.
  • Follows up on assigned referrals from outreach supervisors, social workers, and other staff.
  • Maintains client activity tracking data and enters client information into outreach databases, including written monthly reports documenting client and field work.
  • Provides brief individualized health education related to prevention services, healthy behaviors, and infant mortality prevention.

 

Qualifications/Education

  • High school diploma or equivalent required

 

Experience

  • Minimum one year of experience working with clients in a community agency, healthcare setting, outreach program, or customer service environment preferred

 

Skills and Knowledge

  • Bilingual in Spanish and English (required)
  • Knowledge of community resources and target populations
  • Understanding of social determinants of health and their impact on birth outcomes and infant mortality
  • Strong verbal and written communication skills
  • Ability to build relationships with diverse populations
  • Ability to maintain confidentiality and follow HIPAA regulations
  • Strong organizational and documentation skills

 

Schedule

  • Tues. 8am-5pm, Weds. 8am-5pm, Friday 1pm-5pm
  • 20 hours per week
  • 6-8 week temporary assignment

 

Compensation

  • Starting at $15+/hour, based on experience and tenure
